We are seeking an enthusiastic and experienced Advanced Nurse Practitioner (ANP) to join Glossop Primary Care Network (PCN) for 30 hours per week, working across our four GP practices. This is an exciting opportunity to play a key role in delivering high‑quality, patient‑centred care and supporting accessible, efficient primary care services for our local community.

Working as part of a supportive multidisciplinary team, you will assess, diagnose, treat and manage patients with a wide range of acute and long‑term conditions. By working across all four practices, you will help ensure consistency of care, share clinical expertise and contribute to the PCN’s commitment to integrated, proactive and equitable healthcare. You will undertake clinics within our practices and, when requested by practice, complete home visits within the community.

The Advanced Nurse Practitioner (ANP) provides autonomous, evidence‑based clinical care for patients presenting with acute illness, minor injury and long‑term conditions. Key responsibilities include undertaking comprehensive assessments, diagnosing and managing health conditions, requesting and interpreting diagnostic investigations, prescribing medication where qualified, and developing personalised treatment plans within clinics and home visits.

The ANP works collaboratively within the multidisciplinary team, provides health promotion and preventative care, supports the management of complex patients, contributes to service development and quality improvement initiatives, and maintains accurate clinical records in accordance with professional and organisational standards.

• Provide assessment, diagnosis and treatment at first point of contact by attending to patients according to their needs either in the practice sites or patient homes. Act as an autonomous clinician, with the support and guidance from the GPs.
• Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long‑term condition (as appropriate).
• Diagnose and manage both acute and chronic conditions, integrating drug and non‑drug based treatment methods into a management plan.
• Recognise, assess and refer patients presenting with mental health needs.
• Evaluate clinical information from examination and history taking and initiate appropriate treatment and/or referral, including admission to hospital if required.
