Job Overview

The Respiratory Nurse Specialist will work as a member of the Integrated Respiratory Team in Gloucestershire, within the acute setting to support both inpatient and outpatient services for a variety of conditions such as asthma, COPD, bronchiectasis, ILD, oxygen assessments and providing follow‑up care through the virtual supported discharge service. This is an exciting opportunity for an experienced nurse to step up into a specialist nursing role.

Job Description and Main Responsibilities
  • To provide supported discharge for respiratory patients and facilitate early discharge from the wards
  • To support admission prevention by providing specialist support through in‑reach to unscheduled care.
  • To act as an accessible specialist resource and educator within the Gloucestershire Respiratory Service, providing education and training to colleagues within the trust, and members of primary and community care teams.
  • Proactively participate in quality and service improvement, working closely with the Lead Respiratory Nurse, Senior Matron and multidisciplinary team.
  • Facilitate and support discharge of patients from hospital to home
  • Follow up patients post discharge to prevent inappropriate admission to hospital
  • Participate in the implementation of local policies and procedures to improve the effectiveness of the department
  • To participate in the day‑to‑day co‑ordination of the virtual supported discharge service
  • Respond effectively to referrals within stated timescales
  • Coach patients and carers on how to access help when they feel that their condition or needs have changed, including what to do out of hours
  • Be responsible for data collection and inputting to databases for local and national clinical audits
  • To be involved in monitoring and evaluation of patient feedback, offering recommendations as part of the team to improve services
  • Maintain accurate patient and staff records in line with Trust guidelines.
  • Support the MDT to facilitate safe and effective transition of care between hospital and community services in order to provide seamless support for people.
  • Respond to queries from patients, carers and visitors providing verbal and written information in a timely manner
  • Arrange delivery of consumables to patients when required.
  • Maintain confidentiality at all times in line with Trust and Data Protection policies
  • To be able to prioritise your workload and manage your own time and caseload effectively
  • Good organisational skills and the ability to prioritise effectively
  • To participate in negotiating individual and highly complex care programmes and liaise with other members of the multidisciplinary/multi‑agency team in the planning, delivering and evaluating care to improve outcomes for service users.
