This role is essential for high-quality medicines optimisation, ensuring patients receive optimal care from pre‑assessment to discharge.

You will collaborate with clinical and non‑clinical colleagues, offering expert advice, supporting governance, and enhancing pharmacy services.

• Support medicines optimisation via medication review and prescription review
• Provide expert medicines advice to patients, carers, and staff
• Assist in safe management of medication stock
• Support introduction and evaluation of new medicines
• Contribute to clinical governance, including audits
• Support pharmacy operations including dispensing
• Assist in training colleagues

• Hospital pharmacy experience (preferred)
• Registered Pharmacist with GPhC
• Clinical diploma or equivalent
• Ability to work independently and in a team
