Adult and Children Safeguarding Care Coordinator

The Adult and Children Safeguarding Care Coordinator plays a central role in supporting safe, effective and consistent safeguarding practice across the practice and PCN. The post holder will coordinate safeguarding processes for adults and children, ensuring safeguarding lists are accurate, actions are followed up, information is shared appropriately, and governance requirements are met.

The role supports Safeguarding Leads by maintaining oversight of safeguarding activity, coordinating meetings, audits and reporting, and ensuring compliance with local and national safeguarding guidance. The post holder will work closely with clinical teams, safeguarding leads, social care and partner agencies to ensure safeguarding concerns are identified, recorded, escalated and reviewed in a timely and robust manner.

This is a non‑clinical but essential coordination and governance role within the multidisciplinary team. 20 hours per week – working days must include Tuesday AM, Thursday and Friday.

Main Duties of the Job
  • Maintain accurate and up‑to‑date safeguarding registers and lists for adults and children, ensuring regular review and follow‑up.
  • Coordinate safeguarding processes, including children subject to Child Protection Plans, looked after children, and adults subject to Section 42 safeguarding enquiries or plans.
  • Track and follow up new safeguarding referrals, including MARF, MARAC and MASH referrals, ensuring outcomes and actions are recorded and escalated appropriately.
  • Monitor safeguarding cases pending action, including awaiting MASH outcomes, case conference reviews or escalation decisions.
  • Coordinate regular safeguarding meetings, preparing agendas, triaging cases prior to meetings, and ensuring all relevant internal and external attendees are informed and invited.
  • Record, disseminate and follow up actions from safeguarding meetings and reviews, ensuring accountability and timely completion.
  • Ensure safeguarding coding on clinical systems is accurate and aligned with local safeguarding guidance.
  • Standardise safeguarding reporting processes across the practice and PCN.
  • Support safeguarding audits and assurance work as required.
  • Ensure safe, accurate record keeping using Systm One, including documentation of safeguarding discussions, meetings and reviews.
  • Support Safeguarding Leads by ensuring dedicated time is protected for complex case discussion, reflection and escalation.
  • Disseminate safeguarding information and deliver training to Safeguarding Administrators when processes are updated.
Additional Duties and Compliance
  • Complete mandatory training in line with PCN policy and procedures.
  • Participate in the staff appraisal process.
  • Keep up to date on all matters relating to PCN policy.
  • Adhere to the NHS Constitution, applying the Duty of Candour, infection prevention and control, health and safety, GDPR, confidentiality, risk management, safeguarding duty, and information technology requirements.
  • Discuss significant job changes with the line manager at the time the change occurs and agree on any substantial changes.
Person Specification Experience
  • Experience of clinical medical systems.
  • Experience of working in the NHS and preferably in General Practice.
  • Experience of communication with patients and vulnerable individuals.
  • Experience coordinating safeguarding processes or case tracking.
  • Experience maintaining registers, logs or complex lists.
  • Experience supporting safeguarding audits or reporting.
  • Experience using Systm One or similar clinical systems.
