Main Duties of the Job
  • Offer OT assessment and interventions to individuals with short‑term, recovery‑focused needs, such as significant health episodes with rapid decline in Activities of Daily Living (ADLs).

  • Provide care for recent acute events or hospital discharges with ongoing clinical risk, not on a Home First pathway.
  • Participate in decision‑making and the flow of Occupational Therapy referrals within the locality by supporting clinical triage, ensuring a timely OT response to reduce risk and maintain independence at home.
  • Promote supported self‑management as key to personalised and positive outcomes for people.
Person Specification Qualifications
  • Degree in Occupational Therapy with short specialist courses, experience to a post‑graduate equivalent diploma level.
  • Health Care Professions Council registration (HCPC).

  • Demonstrated specialist knowledge in patients with complex needs and multiple pathologies.
  • Ability to deliver person‑centred, goal‑focused interventions using occupational therapy models of practice.
