The NHS is seeking a Diabetes Care Co-ordinator to join the Gloucester Inner City PCN Diabetes team. You will support the Specialist Diabetes Nurse, identifying complex diabetes patients and ensuring they receive necessary care.
Your responsibilities will include setting up clinics, booking appointments, sending reminders, and following up on missed appointments. The position is part-time at 16 hours a week, fixed term for 2 years, with full training provided for candidates new to GP surgery.
