Emergency Department Specialist Pharmacist, Band 8a (7.5 hours per week)

Join our Emergency Department team as an experienced clinical pharmacist. You will play a key role in developing the clinical pharmacy service, delivering high‑quality pharmaceutical care, improving patient safety, and supporting efficient patient flow.

The post‑holder is responsible for developing and leading the clinical pharmacy service within the Emergency Department.

Key responsibilities include:

• Provide expert pharmaceutical advice and independent prescribing for patients, including medicines reconciliation, optimisation, and discharge prescribing.
• Produce and maintain policies, guidelines and drug utilisation reviews to ensure safe, cost‑effective prescribing.
• Monitor drug expenditure, promote cost‑effective prescribing, and identify opportunities for cost minimisation.
• Act as the central point of contact between the Pharmacy Department and the ED, and participate in late‑night, weekend and bank‑holiday rotas.
• Facilitate discharge medication and support efficient medication flow within the ED.
• Collect and assess medication histories, resolve discrepancies and manage patient medication plans.
• Provide information, education and training to junior doctors, nurses, and other healthcare professionals.
• Engage in audits, research and quality improvement initiatives such as the National Quality Improvement collaborative for Safer Use of Time‑Critical Medicines.
• Represent the pharmacy service at multidisciplinary meetings and in presentations.

Provision of Clinical Pharmacy Service to the ED

• Lead the clinical pharmacy service for patients admitted to the ED, ensuring safe, clinically effective, and cost‑efficient use of medicines.
• Implement and reduce variation in prescribing practice.
• Reduce time for medicines reconciliation for patients transferring to inpatient wards.
• Develop the role of pharmacist non‑medical prescriber within the ED.
• Obtain key pharmaceutical information on newly admitted patients.
• Provide daily clinical pharmacy service within the ED, including medication optimisation, discharge prescriptions and ward rounds.
• Use Sunrise EPR, EMIS and other clinical systems to manage medication information.

Medicines Reconciliation & Optimisation

• Manage independent prescribing qualification to review and prescribe patients’ regular medication.
• Use professional judgement to finalize discharge prescriptions.
• Assist with controlled drug checks and monitoring within the ED.
• Prepare patients for discharge and facilitate efficient prescribing and dispensing.

Expert Pharmaceutical Advice and Service Development

• Collect relevant data and prepare reports for drug usage and expenditure.
• Monitor prescribing to promote rational and cost‑effective prescribing.
• Develop, implement and update policies and procedures related to drug use.
• Represent the pharmacy service and participate in multi‑professional teams.
• Audit and monitor the service and respond to needs.

Prescribing

• Practice within the boundaries of local strategy, professional guidance and legislation when holding a Postgraduate Certificate in Non‑Medical Prescribing.
• Prescribe medicines for agreed patients within competency as a dependent or independent prescriber.

General Duties

• Participate in weekend and bank‑holiday rota as per departmental arrangements.
• Involved in dispensary late‑night rota and lock‑up duties.

Additional Working Pattern Details

• Minimum 5 hours of the 7.5 hours must be worked on Monday.

• Registration with the General Pharmaceutical Council.
• Postgraduate Diploma in Clinical Pharmacy or an equivalent postgraduate qualification.
• 4‑year vocational masters degree with postgraduate competency assessment.
• Completion of an advanced clinical assessment module.
