The Band 3 Community Health Care Worker is required to have a broad range of experience. When working with people living with a learning disability, ensuring reasonable adjustment within the general health care setting both in the community and inpatient areas.

Key Responsibilities
  • Work as a member of the multi‑professional team providing clinical care to patients as delegated by the Registered Nurse and Registered Nurse Associate.
  • Ensure a high standard of patient‑centred care.
  • Promote self‑management of health needs and adopt a positive risk‑based care approach.
  • Complete relevant care forms under the direction of a Registered Clinician.
Clinical Responsibilities
  • Support the implementation of care plans under the supervision of Registered Clinicians.
  • Assist Registered Clinicians in assessment, planning, management and evaluation of care.
  • Perform and record clinical observations.
  • Undertake clinical tasks appropriate to competence such as venepuncture, ECGs, and feedback findings to Registered Clinicians.
  • Accurately record nutritional and fluid intake.
  • Ensure the privacy, dignity and safety of patients at all times.
  • Plan and organise own activity, prioritise workload, and feed back to the team.
  • Input all activity with service users into clinical systems for accurate statistics.
  • Recognise safeguarding issues and report concerns to appropriate registered care professionals.
  • Assist in implementing appropriate actions to meet patients’ physical, emotional, psychological, social, cultural and spiritual needs.
  • Demonstrate understanding of caring for patients with mental illness, learning disabilities and autism.
  • Signpost patients to services that support their wellbeing, promote independence and community connections.
Professional Competencies
  • Recognise changing priorities and seek advice from Registered Clinicians.
  • Understand all elements of the nursing process and assist in gathering information for assessment, planning, management and evaluation.
  • Report and share information with Registered Clinicians on patient condition, behaviour, activity and responses.
  • Assess environmental risks before providing care in community settings.
  • Demonstrate good understanding of consent principles.
  • Apply the Mental Capacity Act, Mental Health Act and Deprivation of Liberties Act, plus Best Interest principles in everyday practice.
Leadership and Team Collaboration
  • Coach and teach patients, carers and other staff.
  • Assist with the development, implementation and monitoring of clinical standards and outcomes under supervision of a Registered Clinician.
  • Develop knowledge of other providers’ resources and referral systems to meet patients’ needs within practice parameters.
  • Communicate with patients, carers, professional colleagues and voluntary/charitable sector to ensure seamless care.
  • Maintain documentation and records in accordance with Trust policy.
