, we support people to live safely, independently, and comfortably in their own homes. Our compassionate team provides high-quality home care, specialist care, live-in care, and 24-hour support to individuals and families across the local community.

Due to continued growth, we are looking for an
experienced Care Coordinator
to join our friendly team and work alongside our
Care Manager
to ensure the smooth running of our domiciliary care service.

This is an exciting opportunity for someone who is passionate about high-quality care and wants to develop their career in adult social care within a supportive and growing organisation.

The Role:

This role is primarily
office based
, with some community visits and occasional frontline care support when needed.

You will play an important role in ensuring our clients receivesafe, person-centred carewhile supporting our care team to deliver excellent service.

Key responsibilities:
  • Coordinating care visits and supporting the day-to-day running of the service
  • Undertakinginitial assessments for new clients
  • Writing and reviewing
    care plans and risk assessments
  • Conductingcare plan reviews with clients and families
  • Ensuring carers understand and follow care plans
  • Supporting and supervising members of the care team
  • Helping arrange cover for staff sickness and annual leave
  • Participating in the on call rota to support the team when needed
  • Occasionally supporting withfrontline care delivery
What We Are Looking For:

We are looking for someone who is organised, compassionate and passionate about delivering high-quality care.

The ideal candidate will be confident supporting staff, building relationships with clients and families, and ensuring care services run smoothly.

Essential requirements:
  • Full UK driving licence and access to a vehicle
  • Level 3 Diploma in Health & Social Care
    (or willing to work towards)
  • Minimum2 years’ experience in a supervisory role within domiciliary care
  • Good understanding of
    CQC regulations and care standards
  • Strong communication and organisational skills
  • Ability to lead and support a team

A willingness to work towards
Level 5 Diploma in Leadership for Health & Social Care
would be advantageous.

Working Pattern:
  • 40 hours per week
  • Combination ofoffice work and community visits
  • Participation in
    on call rota
  • One weekend on / one weekend offsupporting care delivery when required
