We are excited to launch our new countywide Home First service on 1 September 2026 and are looking for enthusiastic and motivated Therapy Assistants to join our growing team.

Working alongside Occupational Therapists, Physiotherapists and the wider multidisciplinary team, you will support people in their own homes following hospital discharge, helping them regain confidence, maximise independence and achieve their personal goals. You will manage your own delegated caseload, delivering person-centred therapy and reablement interventions, undertaking assessments within your competency framework, and supporting people to live safely and independently at home.

As a Therapy Assistant within the Home First service, you will support people in their own homes following hospital discharge to maximise independence, recovery and wellbeing. Working as part of a multidisciplinary team alongside Occupational Therapists, Physiotherapists and other health and social care professionals, you will manage a delegated caseload and deliver person-centred therapy and reablement interventions within your competency framework.

You will undertake holistic assessments, monitor progress, and adapt interventions to support individuals to achieve their goals and maintain independence  role includes supporting mobility, daily living activities, functional rehabilitation, risk assessment, and the provision of equipment and minor adaptations where appropriate.

You will provide advice and education to individuals, families and carers, maintain accurate clinical records, contribute to discharge planning, and work closely with colleagues and partner organisations to ensure coordinated care.

The post holder will participate in supervision, training and continuous professional development, contribute to service improvement initiatives, and support the development of less experienced staff, students and learners where appropriate.
