Clinical Pharmacist

• salary up to £50,000 (pro rata, based on 18 hours per week) 25 days annual leave plus bank holidays, pro rata.

• Support medicines optimisation through medication review, reconciliation and prescription review.
• Provide expert medicines advice to patients, carers, consultants and staff.
• Assist with safe procurement, management and control of medication stock.
• Contribute to clinical governance, audits, incident reporting and the introduction of new medicines and therapies.

You are a registered Pharmacist with the GPhC. You have hospital pharmacy experience and a clinical diploma or equivalent experience. You are an independent prescriber or have experience in secondary care pharmacy.
