PositionSummary
We are seeking a skilled and compassionate Admissions Nurse (RN) to serve as the face of our hospice team across communities in the Greater Twin Cities. This road‑warrior position is ideal for nurses with prior hospice, oncology, or palliative care experience, either directly in hospice or in acute‑care settings with patients nearing end‑of‑life.
Key Responsibilities- Travel to patient residences and senior communities to complete hospice admissions.
- Assess clinical eligibility and initiate services with warmth, clarity, and professionalism.
- Educate patients and families on hospice philosophy, benefits, and next steps.
- Collaborate with physicians, intake coordinators, and interdisciplinary teams.
- Document all clinical findings and care coordination accurately and promptly.
- Active RN license in the State of Minnesota.
- Experience in hospice, oncology, or palliative care required (home care or hospital).
- Comfort discussing end‑of‑life care with patients and families.
- Strong clinical assessment and communication skills.
- Valid driver’s license and reliable transportation.
