Position Summary: We are seeking a dedicated Home Health Occupational Therapist to assess and evaluate patient care needs—functionality, activities of daily living, fine motor coordination, home assessments, and adaptive equipment—to improve patient outcomes.

• Provide occupational therapy evaluations and services in clients’ homes.
• Administer diagnostic evaluations and treatments to increase functioning.
• Help each client reach maximum performance for goals and outcomes.

Qualifications:

• Current Occupational Therapist License in the State of Minnesota.
• Valid driver’s license and reliable vehicle.
• Demonstrated ability to work autonomously.
• Experience with electronic medical records (EMR) software is preferred.

Military Occupational Classification (MOC) Codes:

• Army: Occupational Therapy Specialist (MOS 68L)
• Air Force: Physical Medicine Specialist (4J0X2)
• Marines: Hospital Corpsman (HM)
