Monarch Healthcare Management is seeking an LPN to provide residents with high-quality direct routine care under the supervision of the RN and in accordance with the Minnesota BON standards, the Resident’s Bill of Rights, and the facility QAPI program.

The LPN participates in the development and modification of the resident Plan of Care, implementing interventions delegated, ordered, or prescribed by the RN or physician. The LPN collects resident health data, compares findings to normal ranges and current health status, and reports changes and responses to interventions to the RN. The LPN assigns and monitors nursing tasks or activities of unlicensed staff based on assessed knowledge, skills, and capabilities, clarifying expectations and addressing behaviors or escalation when needed.

All nursing activities support residents in achieving and maintaining the highest practicable physical, mental, and psychosocial well-being.

• Completion of a professional nursing education program approved by the Minnesota Board of Nursing
• Current Minnesota License as a Practical Nurse
• Current Certification in Cardiopulmonary Resuscitation (CPR) and emergency care
• Experience in skilled nursing and/or assisted living facilities
