Position SummaryWe are seeking a Nurse Practitioner to join our growing care team and provide the best care to residents within all senior living communities. If you enjoy long-term relationships with patients, autonomy, and changing environments, apply today!
Wound care certification is highly desired, but not required—we will pay for you to obtain it if you do not already have it.
Responsibilities- Drive to Senior Living Communities to provide on-site primary care to residents
- Facilitate the patient’s plan of care: chronic care management, advanced treatment, and end‑of‑life planning
- Coordinate with the interdisciplinary team, reconcile medications, and order labs
- Communicate clearly and regularly with Physician, office support staff, community staff, and patients’ families
- Licensed as a Certified Nurse Practitioner / CNP in the State of Minnesota
- Experience in Geriatrics and/or Primary Care
- Valid driver’s license and current auto insurance
- Passion for serving the senior population
- Strong interpersonal communication skills
