The Colorado Department of Revenue is hiring a Lead Mid-Level Provider / Nurse Practitioner to serve the Lookout Mountain Youth Services Center. Department Information

The Division of Youth Services (DYS) provides a continuum of residential and non‑residential services, including juvenile detention, commitment, and parole. DYS operates fifteen secure facilities for youth ages 10 to 21, serving pre‑adjudicated, sentenced, or committed youth. Job Overview

Work Schedule: Sunday–Saturday, flexible shifts including possible weekends. Hourly commitments are determined during interview and based on facility needs.

Purpose of This Position: Deliver comprehensive health care to detained, pre‑trial, and committed youth, providing primary care, psychiatric, and dental maintenance through a continuum of services.

Responsibilities
  • Act as the facility’s Responsible Health Authority (RHA) and oversee health screens, appraisals, physicals, and SOAP notes.
  • Supervise and review youth health assessments, monitor communicable disease detection, and report to the CDC/State Health Department as required.
  • Serve as the facility’s lead decision‑maker during emergency medical situations and monitor the effectiveness of treatment plans.
  • Accept and manage pharmacy orders, coordinate with pharmacy staff, and ensure accurate medication records.
  • Communicate health information to facility staff, families, supervisors, and administration to promote optimal youth care.
  • Provide on‑site support for medical case complexity and direct referrals to specialists within a fiscally mindful framework.
  • Lead the medical liaison with the mental health team, including staff training on mental status assessments and coordination of mental health referrals.
  • Supervise staff documentation and practice, including histories, exams, diagnoses, and EHR entries.
  • Conduct performance reviews, address staff conflicts, and manage staff scheduling, hiring, and training.
  • Collect and report monthly data for Behavioral Health and Medical Services, and review policies with facility administrators.
  • Perform physical exams, order labs and diagnostics, formulate treatment plans, and provide health education to youth and staff.
  • Manage specialty care referrals, transportation, and follow‑up on community providers.
  • Participate in committees such as facility management, medical leadership, mental health, safety, and EHR.
Minimum Qualifications
  • Current, valid licensure as an Advanced Practice Nurse (Colorado Board of Nursing) or Physician Assistant (Colorado Medical Board).
  • Prescriptive authority – “RXN status” (per DORA).
Preferred Qualifications
  • Two or more years of experience with prescriptive authority in Colorado.
  • Experience in adolescent healthcare, including diagnosis and treatment of acute/chronic illness.
  • Proficiency in electronic medical record documentation.
  • Experience as a state of Colorado employee.
Additional Competencies
  • Knowledge of communicable disease prevention and emergency medical assessment.
  • Strong oral and written communication skills.
  • Conflict resolution and interpersonal skills.
  • Clinical supervision experience.
Conditions of Employment
  • Minimum age 21 years.
  • Comprehensive background and drug screening.
  • State of Colorado residency at the time of application and during employment.
  • Valid Colorado driver’s license; may be required for vehicle use.
  • Ability to obtain a Colorado Advanced Practice Nurse or Physician Assistant license within 12 months of hire.
  • Shift work across 24/7 facility; may require evenings and weekend coverage.
  • Completion of DYS Pre‑Service Training Academy, Field Training Program, and mandatory annual trainings.
  • Participation in the Diana Screen® for safety.
