Qualifications
  • B.S., Doctorate, MA, or MS in Physical Therapy from an accredited university recognized by the APTA.
  • Current state license and/or registration in applicable state.
Job Responsibilities
  • Provide comprehensive physical therapy evaluations based on MD orders.
  • Document findings on standardized evaluation forms accurately and in adherence to guidelines.
  • Develop comprehensive treatment plans with goals, frequency, duration, and modalities.
  • Obtain MD orders for evaluations, treatments, re-certifications, and discharges.
  • Use modalities and modify treatment as indicated while adhering to precautions.
  • Maintain daily participation and progress notes on weekly progress notes accurately and timely.
  • Update treatment plans and discharge summaries as indicated.
  • Complete required documentation forms and maintain MDS info as needed.
  • Attend facility and rehab meetings, committee meetings, and interdisciplinary team meetings as assigned.
  • Support scheduling of patients and perform patient screens.
  • Accurately bill patients for services rendered.
  • Cover other therapists’ caseload during absences.
  • Supervise junior staff or students, co-sign notes, and provide education to staff and community.
  • Comply with facility policies, patient confidentiality, federal resident rights.
  • Maintain professional relationships, rapport, and positive workplace culture.
  • Adhere to productivity requirements and facility HR policies.
  • Perform any other duties assigned by supervisor, lead therapist, or regional manager.
