In keeping with the Abrazo Health Care core values, the Clinical Pharmacy Specialist will develop, implement, and evaluate clinical pharmacy services for the facility, participate in patient care, provide education, and collaborate with multidisciplinary teams.
Responsibilities- Participates in critical care rounds, optimizing drug therapy
- Optimizes drug therapy and participates in clinical pharmacy services (i.e. pharmacokinetics service) in acute care areas (ED, ICU, Hospitalists' Service)
- Develops evidence-based best practices, including order sets, conducts quality improvement projects, and designs and executes medication use evaluations in areas of need
- Provides education to medical, nursing and pharmacy staff on clinical pharmacy initiatives
- Assists the Director of Pharmacy with the organization of and presentations for the Pharmacy and Therapeutics Committee meetings
- Other related job tasks or responsibilities as assigned
- Education:
- Bachelor of Science, Pharmacy Degree
- BLS
- Experience:
Pharmacy practice residency or equivalent clinical experience - Licensure/
Certifications:
Current Pharmacist license in the State of Arizona/Arizona Pharmacy License
- Education:
Doctor of Pharmacy Degree - Experience:
Critical Care residency - Certification/Registration: ACLS, BCPS
