Position Summary

Position Details

• Status:
  Full‑time, Monday‑Friday, 8:00 AM-5:00 PM
• Compensation: $85 K+ annually
• Work Area:
  West Valley (Glendale / Goodyear)

Your Role

• Conduct comprehensive physical, social, and psychological assessments.
• Collaborate with patients, families, and the interdisciplinary group to create individualized care plans.
• Manage care delivery and maintain meticulous patient records.
• Act as a resource for families, make proactive referrals, and foster a holistic care experience.

Qualifications

• Active Registered Nurse (RN) license in Arizona.
• Previous Home Health or Hospice experience (preferred).
• Effective verbal, written, and time‑management skills.
• Valid driver’s license, reliable transportation, and proof of current auto insurance.
• Ability to perform patient transfers with the full range of movement.
• Current CPR certification and negative TB test (we can assist).
• Valid Arizona Fingerprint Clearance Card (we can help obtain it).
