About the role
An exciting opportunity has arisen for a Practice Nurse to join our very stable and long‑standing nursing team. The post holder will work as an autonomous practitioner within the multidisciplinary team, delivering high‑quality, evidence‑based nursing care to the practice population. The role involves managing acute and long‑term conditions, promoting health and well‑being, and supporting the delivery of safe, effective, patient‑centred services.
We are based in the market town of Howden, close to Goole and easily accessible from Hull and surrounding villages. The position is part‑time at 30 hours per week and is ideal for the right person given our current list size of 8300, expected to almost double over the next decade, and the planned new practice building.
Key Responsibilities- Undertake comprehensive patient assessments and formulate appropriate treatment plans.
- Manage and monitor patients with long‑term conditions.
- Provide treatment‑room services including wound care, venepuncture, ECGs, cervical cytology, injections and immunisations.
- Deliver NHS Health Checks, health promotion and preventative care initiatives.
- Request, interpret and act upon investigations and blood test results within scope of practice.
- Provide health education and support patients to self‑manage their conditions.
- Identify and manage patients at risk of developing long‑term conditions.
- Provide clinical advice and support to the multidisciplinary team.
- Deputise for the Lead Nurse / Nurse Manager when required.
- Participate in clinical meetings, audits and quality improvement initiatives.
- Assist in the development and review of clinical policies, protocols and procedures.
- Ensure compliance with CQC standards, infection prevention and control policies and safeguarding procedures.
- Maintain accurate patient records and uphold confidentiality and information governance requirements.
- Maintain current NMC registration and practise in accordance with the NMC Code.
- Participate in appraisal, mandatory training and continuing professional development.
- Work in accordance with practice policies and Health and Safety legislation.
- Respond to patient feedback and contribute to local quality improvement initiatives.
- Support the effective management of complaints, learning events and risk management processes.
- Act as a positive clinical role model and promote high standards of patient care.
- Support compliance with CQC standards, clinical governance and infection prevention policies.
- Maintain accurate clinical records and contribute to the development of policies and procedures.
- Promote and support safeguarding processes for adults and children.
- Recognise and elevate safeguarding concerns in accordance with practice policies and procedures.
