Floor Nurse

Job Summary

Maple City Health Care Center is a Federally Qualified Health Center dedicated to improving community health by providing comprehensive, accessible care. The Floor Nurse supports patient well‑being through collaborative, multidisciplinary care and patient empowerment.

• Collaborate with the cross‑disciplinary care team to address patients’ holistic needs.
• Engage patients via phone/MyChart/in‑person, utilizing Motivational Interviewing and Trauma‑Informed Care approaches.
• Complete documentation in a timely, clear manner.
• Refill patient medications.
• Complete patient forms and letters in collaboration with providers.
• Co‑coordinate with outside entities for patient care.
• Maintain skill competency by participating in scheduled trainings.
• Perform other duties as assigned to support shared work.

• Spanish/English preferred

• Demonstrates effective interpersonal skills.

• BSN preferred; RN required.
• Indiana RN licensure required.
• Basic Life Support Certification required.
• Trauma‑Informed Care training preferred; training supported.
• Motivational Interviewing training preferred; training supported.

• Lifts up to 20 lbs.
• Prolonged standing, walking, reaching, stooping, bending, kneeling, crouching.
• Prolonged computer use.
