Make a Difference in Diabetes Care
Blossom Health Gosport is seeking an experienced and enthusiastic Diabetic Nurse to join our growing multidisciplinary team at our Gosport site for 20 hours per week
.

Blossom Health Gosport is a forward-thinking GP practice serving communities across Gosport. We pride ourselves on providing excellent healthcare through a strong multidisciplinary approach and a culture that values kindness, teamwork and continuous improvement.
Our clinical team includes GPs, Advanced Nurse Practitioners, Practice Nurses, Clinical Pharmacists, Pharmacy Technicians, Healthcare Assistants and Care Coordinators who work together to provide the best possible care for our patients.
The Role
As our Diabetic Nurse, you will be responsible for delivering high-quality diabetes care and supporting patients to manage their long-term conditions effectively.
Key responsibilities include:
- Running diabetes clinics independently
- Monitoring and reviewing patients with Type 1 and Type 2 Diabetes
- Supporting patients with lifestyle changes and self-management
- Initiating and adjusting treatment plans within competence and local guidelines
- Performing annual diabetes reviews
- Providing patient education and health promotion
- Working collaboratively with GPs, pharmacists and other members of the multidisciplinary team
- Supporting quality improvement and achievement of QOF targets
About You
The successful candidate will:
- Be a Registered Nurse with current NMC registration
- Have experience working within Primary Care or a community setting
- Hold a recognised diabetes qualification or have significant experience in diabetes management
- Be confident managing patients with long-term conditions
- Demonstrate excellent communication and interpersonal skills
- Be committed to delivering high-quality, patient-centred care
- Be organised, adaptable and able to work both independently and as part of a team
Experience using GP clinical systems such as EMIS Web would be advantageous but is not essential.

Working Pattern
- 20 hours per week
- Working pattern to be agreed with the successful candidate
- Based primarily at our Gosport site
