Een zorgorganisatie in Gouda zoekt een begeleider thuis voor volwassenen. In deze rol bied je methodische, praktische begeleiding in de thuissituatie. Je onderhoudt contacten met het netwerk van de cliënten en werkt vanuit een herstelgerichte benadering.
