Kwintes regio Midden-Holland zoekt een Zorgbemiddelaar voor 16 uur per week, standplaats Gouda. Je bent een schakel tussen cliënten, zorgteams, gemeenten en verwijzers en draagt bij aan vraag en aanbod.
Je beoordeelt aanmeldingen, verwerkt beschikkingen en bewaakt wachtlijsten. Je coördineert het proces van aanmelding tot uitstroom en denkt mee over passende zorg en samenwerking.
Wij bieden een tijdelijk contract met uitzicht op vervolgbinding en een marktconform salaris volgens CAO GGZ.
