The Life Skills and Transition Center (LSTC) is seeking a Social Worker to join our team in Grafton, North Dakota.
At LSTC, our work is focused on helping adults and youth with intellectual and developmental disabilities (/DD) to stabilize, build skills, and move toward greater independence. As a Social Worker, you will play an important role in helping people successfully transition to the least restrictive setting that meets their individual needs. As a Social Worker, you will be one of the key connections between the people we support, their families and guardians, the LSTC team, community providers, attorneys, and other partners involved in their care.
You will help people and their support networks understand available services, how to access them, and what to expect throughout their time r work will include completing psychosocial assessments, supporting admissions and discharge planning, providing consultation, and coordinating services.
When it is time to begin planning for discharge, you will help people, families and guardians understand their options. This includes providing information about community service providers and the types of support they may offer informed decisions can be made based on the person's individual needs. You will also work directly with community partners to help.
An important part of this role is making sure the details don't fall through the cracks. You will coordinate and help ensure people attend important appointments and meetings, including Supports Intensity Scale (SIS) and Inventory for Client and Agency Planning (ICAP) assessments, regional referrals, court appearances, and guardianship meetings. You will also assist with guardianship renewals, court authorizations for extensions of stay, and other required documentation to ensure important timelines and legal requirements are met.
To thrive in this position, you don't have to know every answer but you should be someone who knows how to bring the right people together to find it. You should be organized, adaptable, and comfortable shifting priorities as situations and individual needs change. You should enjoy working as part of a multidisciplinary team and building relationships with people who may bring different perspectives to the table.
Strong communication skills are essential. You will regularly communicate complex information with guardians, providers, attorneys, and other professionals. Being able to explain information in a way that is clear, respectful, and supportive matters. Most importantly, you should believe that the person receiving services belongs at the center of the planning process and that successful transitions happen when people work together.
To be considered for this position, you must be a Licensed Baccalaureate Social Worker (LBSW) by the ND Board of Social Work Examiners; or a master's degree in social worker and licensure as a Licensed Social Worker by the ND Board of Social Work Examiners.
If no applicants meet the above qualifications, consideration will be given to applicants who have passed their ND Social Work licensure exam and are awaiting their license number.
