Hartford Health Care at Home is seeking a Visiting Licensed Practical Nurse (LPN) to deliver in‑home skilled nursing care across Connecticut. You will collaborate with the interdisciplinary team, educate patients and families, and document care using our advanced technology to support autonomy and recovery.
Requirements include an active CT LPN license, at least 1 year of LPN experience (homecare preferred), a valid driver’s license with reliable transportation, and computer proficiency.
