Sage Group Health Care Staffing is seeking RN Hospice for a 13-week travel / contract position in Grand Blanc, MI. Accepting both local and traveler candidates.

Pay Rate:
Range $1900 to $2080 Weekly Gross pay (Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments

Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri;
No weekend or on-call
- Registered Nurse: 1 year
- Required - Recent/Current Home Health Nursing experience: 1 year – Required
- Recent/Current Hospice Nursing experience: 1 year – Required
- Home Care Home Base(HCHB) charting
- Highly preferred - OASIS experience preferred
- Must have valid
Driver’s License and Personal Auto Insurance - Must be Covid Vaccinated or provide medical or religious exemption
- ASN or an equivalent– Required
- BSN or an equivalent– Highly preferred
- RN License for State applying or Compact if accepted
- Required - BLS Certification from AHA or American Red Cross
- Required
- Provide and coordinate care for assigned hospice patients in comfort of their homes.
- Develop and implement nursing care plans including Start of Care and admissions as assigned.
- Provide Hospice nursing care and services within the standards and scope of nursing practice asdefined within the state Nurse Practice Act regulations, policies, procedures,and protocols, as well as facility and corporate policies and procedures.
- Ensure physicians’orders is legible if taken verbally and transcribed appropriately perpolicy and procedure.
- Ensure patients’status is assessed on an ongoing basis and pertinent information is gathered and documented.
- Notify physicians of significant changes in status.
- Document and complete charting for all patient visits within 24 hours.
- Provide bereavement support to patient families as needed.
