You will meet clients regularly to foster growth in their social skills and independence. Collaboration with case managers is essential for developing effective programs, and you will also manage documentation, medication administration, and support networking opportunities within the community.

Key Responsibilities:

• Support independence and integration for clients

• Meet with participants in diverse environments

• Create programs with case management support

• Document individual development towards community goals

• Drive clients to necessary appointments

Requirements:

• High School Diploma or GED;
Bachelor's preferred

• At least 2 years of relevant experience

• Valid driver’s license with a good record

• Excellent communication abilities

• Calm demeanor in crisis situations
