Trinity Health At Home’s hospice chaplains provide spiritual care within their local ministry, addressing the spiritual needs of patients, participants, residents, families, and colleagues, with focus on spiritual distress, grief support, crisis intervention, and support during significant life changes. As a fully integrated member of the health care team with professional expertise in spiritual care, we work alongside other care givers, responding to the specific spiritual needs of those served.
We develop, conduct, and participate in programs of spiritual care to promote healthy communities and sacred environments while facilitating the inclusion of faith leaders from churches, synagogues, mosques, etc., as appropriate. Reports directly to the Manager of Spiritual Care and may also report to an immediate ministry site supervisor as appropriate. The following essential functions are the core duties of this position.

- Make available, deliver, and facilitate transformative spiritual care experiences to patients, residents, participants, visitors, and all colleagues across the healthcare continuum and the communities served. Honor and respect the religious beliefs, practices, and values of those served within the total healing process.
- Implement systems and processes to assess religious and spiritual needs, including needs for ethical consultation, and provide interventions or referrals based on the assessed needs.
- Make timely, professional observations of apparent outcomes of spiritual care and adjust continued provision as needed.
- Document in the electronic medical record professional spiritual care assessments (e.g., assessment of 'spiritual distress'), interventions provided, and outcomes observed with appropriate care plans.
- Foster the provision of generalized spiritual care to those served by non‑spiritual‑care colleagues via education programs and modeling of practices.
- Promote and implement workplace spirituality programs in collaboration with the Spiritual Care Manager and the Mission Leader.
- Facilitate opportunities for interfaith or specific‑faith meditation, prayer, worship, and participation in religious sacraments, sometimes collaborating with clergy or ministers from outside communities. Contribute to maintaining appropriate sacred space.
- Master’s degree in theology, religion, spiritual care or spirituality, and/or equivalent combination of education and experience.
- One to three years of spiritual care experience, including CPE training. Chaplaincy or spiritual care ministry experience preferred.
- Board Certified Chaplain (BCC) strongly preferred via certification agencies: APC, CASC, NACC, NAJC, with required master’s degree, 4 units (1600 hours) of ACPE accredited education, and faith‑based endorsement.
- Ability to plan, develop, and implement programs for fulfilling departmental objectives in meeting the spiritual needs of patients/families throughout the care continuum and life cycle for improving community health.
- Strong oral and written communication skills in English with excellent interpersonal skills and ability to present effectively before groups; ability to use clinical/medical terminology.
