Job DetailsThe Skilled Rehab and Home Health team at Good Samaritan Grand Island and Hastings seeks to hire a COTA to provide therapy services in Skilled Nursing Facilities and surrounding Home Health clients. The role is full‑time (35‑40 hours weekly, Monday–Friday with flexible weekend/holiday coverage). New graduates are encouraged to apply.
Travel between patients and facilities is paid, with mileage reimbursement.
Job SummaryAssist patients/residents/clients with therapy activities and exercises scheduled in a treatment plan under Occupational Therapist supervision. Provide, monitor, and record therapy services for physical, cognitive, developmental, and psychosocial impairments. Select, implement, and modify therapeutic activities aligned with patient goals and practice standards. Communicate observations and documentation to the supervising therapist. Perform related duties as needed. Focus may include post‑acute, long‑term care, and home health environments.
Qualifications- Appropriate education level in accordance with state licensure requirements.
- Licensed as an Occupational Therapy Assistant in the state(s) of practice.
- When working at a Sanford Health Facility (preferred for GSS Facilities):
Must also be certified as a COTA through NBCOT and comply with supervision and licensing standards of the state(s) where services are provided. - Valid driver’s license may be required.
- Basic Life Support (BLS) certification required within six months of employment.
