Summary
This Licensed Practical Nurse (LPN) position is located at the Grand Island VA Medical Center in Grand Island, Nebraska. The LPN works under the direction of an RN as a member of a Primary Care team, providing basic and specialized nursing care for adult and geriatric patients with a variety of physical, mental, behavioral, or psychological needs.

- Serve as an LPN on a Patient Aligned Care Team (PACT) in Primary Care.
- Assist with intake of Veterans in preparation for provider visits and maintain clinic flow.
- Complete pertinent clinical reminders and preventative health screenings.
- Document all data collected and nursing care provided in the Veterans electronic medical record.
- Assist providers with exams/procedures as needed.
- Perform treatments and procedures such as electrocardiograms, vital signs, weights, blood glucose readings, basic foot care, bladder scans, ear flushes, etc.
- Track and administer required immunizations.
- Administer ordered medications in accordance with Medical Center and Nursing Service policy and monitor for any side effects.
- Participate in the management and follow‑up of patient care needs and requests.
- Initiate procedure preparations, adapting them to patients’ physical and emotional needs before, during and after procedures, observing and managing therapeutic or possible complications resulting from the procedure.
- Initiate and perform nursing measures to prevent complications by recognizing deviations and/or changes in patient’s conditions.
- Recognize and respond/initiates emergency measures. Respond promptly to emergency situations in the clinic including but not limited to administering CPR.
- Perform venipuncture and specimen collection and prepare specimens for storage and/or transport.
- Participate in PACT team huddles and meetings as needed.
- United States Citizenship.
- Active, full, current and unrestricted licensure as a Graduate Licensed Practical or Vocational Nurse in a State, Territory or Commonwealth (e.g., Puerto Rico), of the United States or the District of Columbia.
- Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant.
- Basic Life Support (BLS) certification required. Training provided.
- Basic English language proficiency.
- Experience at GS‑03 – GS‑06 levels as described in the job announcement.

- Participation in the seasonal influenza vaccination program required for all VA Health Care Personnel.
