About this role:

As a
part-time
Advanced Practice Practitioner in theHIV Collaborative Clinic y'syou will work with a team of healthcare professionals committed to providing quality care and excellent patient experience. In this role you will serve as a primary care provider for patients living with HIV/AIDS.

• This position includes a$5,000
  sign-on bonus and up to$3,750
  in relocation assistance when applicable

• Clinic is open Monday through Friday, 8am-5pm

  • FTE: .5 (18 Patient Facing hours, 2 administrative hours) Schedule is Flexible

• Support for AAHIVM certification will be provided

About this Clinic:

The HIV Collaborative Clinic y's has been fully funded through the Ryan White HIV/AIDS Program (RWHAP) since 2000.As a Local community
-based Clinic (Part

C) , we are able provide outpatient ambulatory health services and support for people with HIV/AIDsthroughout 22 counties of Western Colorado. With the help of the RWHAP, we are able to provide care, medication, and support services to improve HIV-related health outcomes and reduce the spread of HIV.

What youll bring:

• APRN or PA-C degree from an accredited program,
  NP preferred

• Minimum of 1-2 years of Primary Care/ Family Medicine, required

• Active, unobstructed
  
  Colorado medical license, or the ability to obtain one

• Full prescriptive authority and an active DEA, or the ability to obtain one

• Ability to successfully complete Intermountain Healths credentialing process
