Full-Time Registered Nurse (RN) – Home Health

Summit West Care | Grand Junction, CO

Pay: $35–$42/hour (DOE)

Schedule: Monday–Friday | 8:00 AM – 5:00 PM + rotating on‑call

• Provide skilled nursing care to patients in their homes
• Complete comprehensive assessments and develop individualized care plans
• Monitor patient progress and adjust care as needed
• Educate patients and families on medications, treatments, and self‑care
• Collaborate with an interdisciplinary team (RNs, LPNs, therapists, caregivers)
• Maintain accurate documentation using Home Care Home Base (HCHB) EMR
• Participate in a rotating on‑call schedule

• Competitive pay: $35–$42/hour (DOE)

• Active Colorado RN license (required)
• Home health experience preferred - training available for the right candidate
