Full-Time Weekday LPN – Deliver Compassionate Care Where It Matters Most

Schedule: Monday–Friday | 8:00 AM – 5:00 PM
Pay: $25–$32/hour (DOE)

• Deliver personalized, hands-on nursing care to patients in their homes

• Perform essential services such as wound care, medication administration, and catheter care

• Monitor and report patient conditions to the RN or clinical team

• Support chronic disease management and educate patients and families

• Ensure accurate, timely documentation using our EMR system

• Travel within Mesa County for local home visits (mileage reimbursed)

• Competitive pay: $25–$30/hour, depending on experience

• Consistent weekday schedule:
  Monday–Friday, 8 AM – 5 PM

• Mileage reimbursement

• Active LPN license in Colorado, IV Certification a plus!

• Home health experience preferred (but not required – we'll train the right person!)
