American Traveler is hiring a Hospice RN to lead an interdisciplinary care team across home, nursing home, and assisted living settings, requiring a CO or compact RN license and 2+ years of experience.
Requirements- Active CO or compact RN license required
- Minimum of 2 years of nursing experience required
- Strong clinical assessment and critical thinking skills required
- Generalist knowledge across disease processes with specialization in symptom management
- Weekday schedule Monday through Friday, 8am to 5pm
- Occasional back-up coverage required for weekends and on-call
- Care provided in patients' homes, nursing homes, group homes, or assisted living facilities
- Primary RN carries an assigned caseload and leads the interdisciplinary care team
- Serve as both clinical caregiver and teacher, helping patients set goals of care and educating caregivers
- Provide compassionate, personalized hospice and palliative care
- Work within a supportive, mission-driven hospice environment
