Are you a compassionate nursing leader who is passionate about improving the lives of older adults? Hope West is seeking an experienced Clinic Manager (RN) to lead the daily operations of our innovative PACE clinic, where you'll guide an exceptional interdisciplinary team and help ensure participants receive coordinated, high-quality care that allows them to remain safely and independently in their homes.

The Clinic Manager oversees the daily operations of the PACE Clinic, ensuring efficient workflows and delivery of high-quality, participant-centered care. This role provides leadership to clinical staff, supports clinical best practices, and ensures compliance with all applicable state and federal regulations. The Clinic Manager partners with interdisciplinary teams to coordinate care, implement clinical policies, and maintain operational excellence aligned with Hope West standards.

PACE (Program of All-Inclusive Care for the Elderly) is a unique Medicare and Medicaid program designed to help older adults remain living safely and independently in their own homes for as long as possible.

Through an interdisciplinary approach, PACE provides comprehensive medical care, rehabilitation, transportation, social services, medications, and other essential supports—all coordinated through one dedicated team.

With more than 200

PACE programs serving over 95,000 participants across 33 states, the model continues to grow as communities recognize the value of coordinated, whole-person care.

As the population of older adults continues to grow in Western Colorado, so does the need for compassionate healthcare leaders who want to make a lasting difference.

Associate's degree in nursing required, bachelor's degree preferred.

2 years experience in healthcare leadership required, 3 years preferred.

1-2 years experience working in a clinic setting highly preferred.

Valid Colorado driver's license, reliable transportation, and required automobile insurance

Current CPR certification maintained throughout employment.
