Hope West is seeking an RN/LPN to join our Hospice Float team in Grand Junction, CO. This part-time role (24 hours/week) provides flexible clinical support across hospice teams, ensuring continuity of care for patients and families.
You will perform assessments, plan care, manage symptoms and medications, and educate families while coordinating with the interdisciplinary team. The position requires an unrestricted Colorado RN license, a valid CO driver’s license, and at least 1 year of nursing
