The Health Coach is responsible for providing health coaching to members, across all brands, with chronic diseases.
Location:
Virtual – This role enables associates to work virtually full-time, with the exception of required in-person training sessions (when indicated), providing maximum flexibility and autonomy. This approach promotes productivity, supports work‑life integration, and ensures essential face‑to‑face onboarding and skill development. Please note that, under our policy on hybrid/virtual work, candidates not within a reasonable commuting distance from the posting location(s) will not be considered for employment, unless accommodation is granted as required by law.
You Will Make An Impact
- Coordinates specific health coaching as directed by nurse case manager to address objectives and goals as identified during assessment.
- Implements coaching plan by using behavior change principles to identify member barriers and develop ways to overcome those barriers.
- Coordinates with the nurse case manager to provide feedback on member goal attainment and clinical issues.
- Identify and address HEDIS/STARS gaps in care.
- Maintains fidelity to the Coleman Model for Care Transitions Intervention.
- Conducts Social Determinants of Health assessments and identifies resources to support member needs.
- Monitors patients as they transition from hospital to home, completing post‑discharge follow‑up, medication reconciliation, reducing members’ overall risk of readmission.
- Requires BA/BS in an appropriate field of specialization (examples such as Health Education, Exercise Physiology, Respiratory Therapy or Dietician) and minimum of 3 years of related experience in health education, exercise instruction or patient education; or any combination of education and experience, which would provide an equivalent background.
- Certification, advanced certification, and/or license appropriate to field of specialty as required.
- Prior experience in health coaching, disease management and knowledge of behavioral and/or clinical assessment techniques, health and/or patient education and behavior change techniques preferred.
- Understanding of disease management principles preferred.
- Current unrestricted LMSW, LCSW, LISW, or LPN (or equivalent) license is needed for this position.
- Care Transitions Intervention Health Coach certification preferred.
- Prior experience in UM and CM highly preferred.
- Bilingual Spanish preferred.
- Candidates from all states are welcome, but they must reside within commuting distance of a Pulse Point office location where we have an office to be considered (travels to worksite, client locations, and other sites as necessary).
