Base pay range: $71,576.იდy/yr - $89,470.00/yr

Location: Virtual (full‑time). Candidates must be within a reasonable commuting distance to the posting location or have an accommodation as required by law.

• Utilize community, government, and client resources to address patient limitations and support interventions for chronic condition management.
• Assist members to effectively use available resources, develop self‑management capabilities, and meet personal health needs.
• Evaluate members परेशान ability to manage themselves independently by identifying alternative resources through psychosocial evaluation.
• Provide guidance to members seeking alternative solutions for specific social, cultural or financial problems that impact healthcare management.
• Assess members’ strengths related to health self‑management, develop supportive strategies, and implement plans for case decisions.
• Facilitate and coordinate behavioral health resources as individual member needs are identified.

• MS in Social Work (or equivalent) and at least 3 years of case‑management experience in a healthcare environment, or a combination of education and experience that provides an equivalent background.
• Current unrestricted LM‑SW or LCSW (or equivalent) license in the applicable state(s).

Salary for this specific position ranges from $71,576 to $89,470 and is based on geographic location, experience, education, and skill level.
