We’re seeking a passionate and driven Physician Assistant/Nurse Practitioner to join our team in Caledonia, Michigan

• Outpatient Department of Trinity Health Grand Rapids Hospital
• Our Caledonia location offers primary care services with a team of Providers that specialize in Family Medicine working together to deliver a comprehensive, personalized patient experience
• Office also offers convenient on-site access to several services that are in the same facility including Lab, Advent Physical Therapy and Grand Rapids Ophthalmology
• Practice currently has a team of 4 Physicians
• 4 days/week schedule with extended hours and no weekends
• No call for APP
• Very close knit & collaborative office with a supportive environment

• Primary Care experience required
• APPs:
  Board certified in specialty prior to employment
• Active Michigan medical license (or ability to obtain)
• DEA license
• ACLS/BLS certification (must be maintained)

A typical day may include:

• Providing direct patient care
• Collaborating with multidisciplinary teams
• Participating in teaching and mentoring
• Documenting in Epic EMR
• Engaging in quality improvement initiatives
• Supporting community health efforts
