- Provide in-home primary care to a medically frail, elderly population - Collaborate within a team-based care model including physicians, advanced practice professionals, RN care managers, social workers, and medical assistants - Serve approximately 830 patients across a 10-county area in West Michigan
• Additional Responsibilities:
- Build and develop your Geriatrics practice with strong clinical and administrative support - Engage in value-based care initiatives with Priority Health, a system-owned health plan - Opportunities for teaching medical students, residents, and fellows Highlights

Qualifications
• Board Eligible/Board Certified in Geriatrics or Internal Medicine with geriatrics experience
• Medical Degree (MD/DO) with licensure in Michigan
• Certification in Basic Life Support (BLS)
