Summary:

Join Curana Health in transforming senior healthcare. We are seeking a compassionate and driven Physician to build and manage a dedicated panel of patients across independent and assisted living facilities. Essential Duties & Responsibilities:
  • Patient Panel Development:
    Build and maintain strong patient relationships by developing a dedicated panel and providing continuity of care.

  • Direct Patient Care:
    Conduct assessments, manage chronic conditions, and deliver preventive services as the primary physician for residents.

  • Routine Rounds:
    Perform regular visits at assigned facilities, addressing both ongoing and acute health needs.

  • Care Coordination:
    Partner with facility staff, families, and interdisciplinary teams to create personalized, holistic care plans.

  • Documentation & Compliance:
    Ensure accurate, timely EMR documentation while adhering to all regulatory standards.

  • Patient & Family Education:
    Provide education on preventive health, chronic condition management, and wellness strategies.

  • Quality Improvement:
    Participate in initiatives that drive better outcomes and elevate the standard of care.

Qualifications:
  • Medical Degree (MD or DO) from an accredited institution
  • Board Certification in Family Medicine, Internal Medicine, Geriatrics, or Hospital Medicine
  • Active, Unrestricted Medical License in the state of practice
  • DEA Certification and current BLS/ACLS certification
  • Proficiency in EMR systems and timely documentation practices
  • Strong Interpersonal and Communication Skills, with a commitment to patient-centered care
