DescriptionPosition Summary: Responsible for providing clinical pharmacy services for inpatient/outpatient patients by providing drug therapy management and assessment for patients in the practice area. Provides support for established programs including drug formulary, pharmacy education programs, and quality improvement and research relating to drug therapy.
Responsibilities- Provide drug therapy management and assessment.
- Identify and resolve potential and actual drug related problems.
- Assist physicians in designing dosing regimens based on patient‑specific parameters.
- Communicate and collaborate with physicians and other clinicians to develop drug therapy policies, protocols, and guidelines.
- Provide support to the Pharmacy & Therapeutics Committee, including preparing formulary drug reviews, medication usage evaluations, and policies.
- Participate in performance improvement initiatives and research as appropriate.
- Participate in departmental, multidisciplinary, hospital, and health‑system committees as leader, member, or consultant.
- Participate in training, education, precepting, and evaluation of pharmacy students and/or residents as assigned.
- Provide education and information on pharmaceutical care and drug therapy to patients, pharmacy staff, physicians, nurses, students and other clinicians.
- Perform other related duties such as quality assurance reports, preparation for regulatory inspections, etc.
- Manage disease programs within approved collaborative practice agreements (diabetes, hypertension, hyperlipidemia, asthma, COPD, anticoagulation, others).
- Conduct referral‑based Comprehensive Medication Reviews with recommendations to physicians.
- Provide daily ad hoc informal consultations to improve pharmacotherapy effectiveness, safety, and cost.
- Participate in team‑based care within a multidisciplinary model.
- Allocate time in Anticoagulation Management Service (~1 day/week) and other high‑risk patient initiatives (~½ day/week).
- Participate in training of Trinity Health Grand Rapids PGY‑1 & PGY‑2 Pharmacy residents and Pharm
D students from Ferris State University. - Engage in medical group and/or health‑system committees.
- Doctor of Pharmacy degree (Pharm
D) from an ACPE‑accredited school/college of Pharmacy (required). - PGY‑1 Residency (required).
- PGY‑2 Ambulatory Care Residency strongly preferred or commensurate practice experience in ambulatory care with direct pharmacotherapy management responsibilities.
- BPS Specialty Certification (Pharmacotherapy or Ambulatory Care) preferred or eligible to obtain within 1 year of hire.
- Licensed pharmacist and controlled substance licensure in the State of Michigan, or eligible.
