Critical Illness Recovery Hospital (LTACH) – Select Specialty Hospital – Grand Rapids, MI
OverviewAs a Clinical Liaison, you advocate for referred patients in the pre‑admission process, collaborate with clinical teams to assess patient needs, foster relationships within medical facilities, coordinate care plans, facilitate patient transfers, and address barriers to accessing specialized post‑ICU care.
Key Responsibilities- Advocate for referred patients in the pre‑admission process by collaborating with clinical teams to assess patient needs.
- Serve as a resource for healthcare professionals by fostering relationships within medical facilities.
- Coordinate care plans with physicians and families and facilitate patient transfers.
Address barriers to accessing specialized post‑ICU care.
- Registered Nurse (RN) or Respiratory Therapist (RT) license.
- Ability to complete clinical assessments of medically complex patients.
- Demonstrated patient advocacy and superior communication and relationship‑building skills.
- Prior healthcare marketing or business development experience is preferred; training provided.
- Full‑time Monday‑Friday 8‑5 pm.
