Hospice Licensed Practical Nurse – Optimal Care

Optimal Care is a clinician‑owned and operated company dedicated to exceptional patient care. We are a leading provider of physician services, home health, and hospice care in the Midwest.

We are looking for an experienced Licensed Practical/Vocational Nurse to join our hospice team.

Key Responsibilities
  • Provide direct patient care under RN supervision.
  • Follow the plan of care and provide treatments, including specialized end‑of‑life care.
  • Perform comprehensive patient assessments and implement nursing practice as per the plan of care.
  • Document patient services accurately and timely.
  • Coordinate hospice services and report identified needs to the interdisciplinary group (hospice aide, OT, PT, SW, ST, Dietician, Clinical Supervisor).
  • Manage pain/symptom treatment and assess patient response to medications.
  • Maintain continuous communication and coordination with physicians, facility staff, and team members.
Required Qualifications
  • Graduate of an accredited practical nurse or vocational nursing program.
  • Currently licensed as an LPN/LVN in the state.
  • Minimum 1 year clinical nursing experience.
Desired Qualifications
  • Experience in hospice or home health environment preferred.
Location

770 Kenmoor Ave, Suite 100, Grand Rapids, MI 49546 (main service area: Greater Grand Rapids, Lakeshore, surrounding).

Hours
  • Office hours: 8:00 am – 5:00 pm, Monday through Friday.
  • Weekend rotation available.
Pay Range

$24–$28 USD per hour.
