Full-Time Home Health Licensed Practical Nurse (LPN)

Looking for a Full-Time LPN to service the Down Grand Rapids, Kentwood and Wyoming, MI Area.

As a Home Health LPN at Trinity Health At Home you’ll provide skilled nursing care to the organization’s clients in accordance with the plan of care as prescribed by the physician and assigned by the supervising registered nurse.

• Graduate of an accredited or state approved school of practical nursing.
• Licensure as an LPN in the State of Michigan.
• Minimum of one year of experience in an acute care setting; home care experience preferred.
• Phlebotomy and wound care experience required.
• Must have passed NAPNS or equivalent medication exam before administering medications.
• Must possess interpersonal skills to interact effectively with clients and families as well as interdisciplinary caregivers, peers, subordinates and supervisors.
• Must have current driver’s license and reliable transportation.
