Overview

Full-Time Home Health Licensed Practical Nurse, LPN - Grand Rapids. Join Trinity Health At Home as a Home Health LPN and provide skilled nursing care to clients in accordance with the plan of care prescribed by the physician and assigned by the supervising registered nurse.

• Provide skilled nursing care to clients in their home per the plan of care.
• Collaborate with the supervising RN and interdisciplinary team to ensure quality patient care.
• Document care using the Epic EMR system to support streamlined documentation and communication.
• Deliver one-on-one care that positively impacts patient outcomes.
• Maintain patient safety and comply with all regulatory and organizational guidelines.

• Graduate of an accredited or state-approved school of practical nursing.
• Licensure as an LPN in the State of Michigan.
• Minimum of one (1) year of experience in an acute care setting; home care experience preferred.
• Phlebotomy and wound care experience required.
• Must have passed the National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
• Interpersonal skills sufficient to interact effectively with clients, their families, and all caregivers and supervisors.
• Current driver’s license and reliable transportation to and from the work site.
