Grand Rapids, MI | AM Shift | 13‑Week Contract

We are seeking an experienced Home Health Registered Nurse for a full‑time, 13‑week assignment based out of Grand Rapids, Michigan. This role offers consistent daytime hours and the opportunity to provide high‑quality care across several established coverage areas.

• Orientation – Branch location:
  Grand Rapids, MI

• Home Health experience required
• Proficiency with HCHB (Homecare Homebase)
• OASIS experience required
• Strong clinical assessment skills
• Ability to work independently in the field
• Reliable transportation

• Provide skilled nursing care to patients in their homes
• Complete OASIS assessments and documentation accurately and on time
• Develop and implement individualized care plans
• Collaborate with interdisciplinary team members
• Educate patients and families on care plans, medications, and safety
• Maintain a high standard of professionalism and patient-centered care
