Key Responsibilities

The Hospice RN Case Manager is a trained professional nurse who administers skilled nursing care to patients and families who are terminally ill and meet hospice admission criteria in the home or facility setting. The role may require performing admission, routine, respite, or continuous care visits and assessing patients within regulatory requirements for hospice. The position is responsible for overall coordination of the care delivered to their patient caseload by making regular visits to patients wherever they reside, communicating regularly with the interdisciplinary team and hospice medical director, identifying patient, family, caregiver needs and providing those needs in accordance with the attending physician's orders and the IDG plan of care.

This position requires the ability to build rapport and communicate effectively with patients, families, physicians, facilities, and other IDG team members.

In this role you will be responsible for:

  • Completing initial evaluation and re‑evaluating the patient’s nursing needs during each visit.
  • Assuming primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the patient plan of care.
  • Completing assessments specific to the provision of hospice care, including but not limited to identification of home safety risks, appropriateness of home environment to the delivery of care, availability and capacity of support systems and community resources.
  • Providing appropriate support at the time of death and performing bereavement assessment if requested.
  • Providing positive, supportive communication to physicians, patients, families, facilities, visitors, and other organization personnel.

Required Qualifications

  • Graduate of an accredited diploma, associate or baccalaureate school of nursing.
  • Current state license as a registered nurse.
  • Minimum 1 year of experience as a nurse in a clinical care setting.
  • Nursing skills as defined by accepted nursing standards.
  • Demonstrates ability to assess and respond to the needs of patients, families, and caregivers in varied settings and to function effectively in the home or nursing facility environment.
  • Demonstrates skill in nursing practice, ability to cope with ill, disabled, terminal patients, family emotional stress, and tolerance of individual lifestyles.
  • Demonstrates ability to handle multiple priorities, documentation requirements, and deadlines.
  • Reliable transportation and valid automobile insurance coverage.

Desired Qualifications

  • BSN preferred.
  • Hospice or home health experience preferred.

    8:00 am – 5:00 pm, Monday through Friday.

Pay Range

$70,000 - $85,000 USD
