Astera Cancer Care seeks a Sickle Cell Program Coordinator (RN) to lead patient-centered care coordination for individuals living with sickle cell disease. You will serve as a clinical liaison, educator, advocate, and navigator across the care continuum.

Collaborating with hematology, primary care, ED, infusion services, social work, and community partners, you will improve access, reduce hospitalizations, and enhance quality of life for patients and families.
